Return to Work Certificate

A return to work certificate, also called a fitness for work or medical clearance note, confirms that you are well enough to go back to your job. It is different from a regular doctor's note. Before we can write one, you need a full visit with a TelePlus Care provider, who reviews your recovery and the demands of your job and decides whether you are ready. If the provider approves your return, the certificate costs $60. If you are not cleared, you are not charged for the certificate.

Quick Answer

A TelePlus Care return to work certificate is issued only after a full assessment visit with a provider. The visit is booked and billed like a regular appointment. If the provider clears you to return to work, the certificate fee is $60. If you are not cleared, there is no certificate fee.

  • A full assessment visit with a provider is required first. We do not issue a return to work certificate without one.
  • The certificate fee is $60, charged only if the provider approves your return to work.
  • The assessment visit is billed like a regular appointment: covered for Alberta patients with an active Alberta Health Card, otherwise $80.
  • If approved, we email you a secure payment link for the $60 fee, and your signed certificate follows once it is paid.

Clinical content is reviewed for accuracy and scope. TelePlus Care appointments go ahead only when a doctor agrees it is the right call, after a real assessment, and the doctor will explain the next step if virtual care is not the right fit.

What a Return to Work Certificate Is

A regular doctor's note explains why you were away. A return to work certificate answers a different question: are you fit to go back? Employers often ask for one after a longer illness, an injury, surgery, or before you return to safety-sensitive work such as driving, operating equipment or heavy lifting. The certificate states the date you can return and, when needed, any temporary limits on your duties.

  • Confirms you are medically fit to return to work
  • States your return date
  • Can list temporary restrictions, such as lighter duties or reduced hours, when appropriate
  • Separate from, and in addition to, a regular sick note

How It Works: Visit First, Then the Certificate

Clearing someone to return to work is a medical decision, so it always starts with a full visit. The provider needs to understand what happened, how you are recovering, and what your job asks of you before deciding.

  • Step 1: Book a visit online and choose "Return to Work Certificate".
  • Step 2: Meet the provider by secure video or phone. They review your illness or injury, your recovery and your job duties.
  • Step 3: The provider decides whether you are fit to return, and whether any restrictions are needed.
  • Step 4: If you are approved, we email you a secure link to pay the $60 certificate fee.
  • Step 5: Once it is paid, your signed certificate is emailed to you.

Cost

There are two separate charges, and you only pay the second one if you are cleared.

  • Assessment visit: billed like a regular appointment. Covered for Alberta patients with an active Alberta Health Card, otherwise $80.
  • Return to work certificate: $60, charged only if the provider approves your return to work.
  • Not approved: no certificate fee. The provider explains why and what to do next.

If the Provider Does Not Clear You Yet

Sometimes it is too early to go back, or the provider needs more information, such as a specialist report, test results or an in-person exam. In that case you are not charged the $60 certificate fee. The provider explains the reason and the next step, which may be more time off, a follow-up visit, or seeing a doctor in person.

What to Have Ready for Your Visit

Having these ready helps the provider make a decision in one visit.

  • The reason you were off work and when it started
  • Any hospital, specialist or test reports you have
  • A short description of your job duties, especially physical or safety-sensitive tasks
  • Your employer's form, if they gave you one
