- Methimazole (Tapazole) — first-line for hyperthyroidism (Graves disease, toxic nodule)
- Propylthiouracil (PTU) — first trimester of pregnancy for hyperthyroidism
- Beta blockers (propranolol, atenolol) — short-term symptom control in hyperthyroid until definitive treatment

Bloodwork — What Gets Ordered & When
Thyroid management is bloodwork-driven. Your doctor orders these through Alberta Precision Laboratories or DynaLIFE — secure virtual care.
- Initial assessment: TSH + available T4 (available T3 if hyperthyroid suspected)
- After starting levothyroxine or adjusting dose: TSH (+/- available T4) at 6–8 weeks — this is when steady state is reached
- Stable on dose: TSH annually, more often in pregnancy
- Pregnancy: TSH every 4 weeks in first half, every 6–8 weeks second half
- Antibody testing (anti-TPO, anti-TG) — useful at initial assessment to confirm Hashimoto's or Graves
- TSI / TRAb — for confirming Graves disease
- Thyroid ultrasound (AHS imaging requisition) — for any palpable nodule or asymmetry
- Important: take levothyroxine at the same time of day, with water, 30 minutes before food and 4 hours away from iron, calcium, biotin, and PPIs — these affect absorption and lab readings

- Graves disease needing radioactive iodine or thyroidectomy assessment
- Toxic multinodular goitre
- Suspected thyroid nodule needing fine-needle aspiration

- Difficult-to-control hypothyroidism on combination T4/T3
- Pregnancy with newly diagnosed thyroid disease
