What We Prescribe (and When We Don't)

- Amoxicillin — equivalent first-line, often preferred for kids (better taste, once-daily dosing)
- Azithromycin (Z-Pak) — for true penicillin allergy (note: rising resistance in some Canadian regions)
- Clindamycin or cephalexin — for non-anaphylactic penicillin allergy or recurrent strep failures
- Symptomatic care for viral sore throat — saltwater gargle, lozenges, acetaminophen or ibuprofen for pain and fever
- Throat swab requisition for confirmation when the picture is unclear
- We do NOT prescribe antibiotics for typical viral pharyngitis (cough + runny nose + mild sore throat) — they cause harm without benefit
Red Flags — When to Go to ED
Some sore throats hide dangerous problems. Go to ED or call 911 if any of the following apply:
- Difficulty breathing or drooling (cannot swallow your own saliva)
- Muffled "hot potato" voice with severe throat pain — possible peritonsillar abscess
- Severe one-sided throat pain with neck swelling
- Stridor (high-pitched noise on breathing in)
- High fever with rigors and severe systemic symptoms
- Inability to open the mouth fully (trismus)
- Suspected epiglottitis — sudden severe sore throat with high fever and breathing trouble
- Sore throat after recent dental procedure with worsening neck swelling

Throat Swabs in Alberta
