Who This Service Is For
Most adult and adolescent sore throats can be assessed virtually. Book if any of these apply:
- Sudden severe sore throat with difficulty swallowing
- Sore throat with fever, no cough — classic strep pattern
- Visible white patches on tonsils
- Sore throat in someone with a recent strep contact at home, school, or workplace
- Recurrent strep — needs assessment and possibly ENT referral
- Sore throat that hasn't improved after 5 days
- Child with sore throat — parent present, will arrange throat swab if needed
- Need a sick note for work or school due to sore throat
What We Prescribe (and When We Don't)
We follow Bugs & Drugs Alberta and Canadian Paediatric Society guidance. Antibiotics are only used when bacterial strep is suspected or confirmed.
- Penicillin V — first-line for confirmed or strongly suspected strep, 10-day course
- Amoxicillin — equivalent first-line, often preferred for kids (better taste, once-daily dosing)
- Azithromycin (Z-Pak) — for true penicillin allergy (note: rising resistance in some Canadian regions)
- Clindamycin or cephalexin — for non-anaphylactic penicillin allergy or recurrent strep failures
- Symptomatic care for viral sore throat — saltwater gargle, lozenges, acetaminophen or ibuprofen for pain and fever
- Throat swab requisition for confirmation when the picture is unclear
- We do NOT prescribe antibiotics for typical viral pharyngitis (cough + runny nose + mild sore throat) — they cause harm without benefit
Red Flags — When to Go to ED
Some sore throats hide dangerous problems. Go to ED or call 911 if any of the following apply:
- Difficulty breathing or drooling (cannot swallow your own saliva)
- Muffled "hot potato" voice with severe throat pain — possible peritonsillar abscess
- Severe one-sided throat pain with neck swelling
- Stridor (high-pitched noise on breathing in)
- High fever with rigors and severe systemic symptoms
- Inability to open the mouth fully (trismus)
- Suspected epiglottitis — sudden severe sore throat with high fever and breathing trouble
- Sore throat after recent dental procedure with worsening neck swelling
How the Virtual Visit Works
Three steps. A clear photo of the throat (open mouth, light shone in) helps your doctor assess for tonsillar exudate.
- Step 1 — Book a slot and complete the intake covering symptom duration, fever, cough, swollen glands, recent strep contacts, and any history of recurrent strep
- Step 2 — Meet your licensed doctor by secure video. They calculate the Centor score, ask you to open your mouth on camera, and decide whether to swab or treat empirically
- Step 3 — Receive your prescription or throat swab requisition. Antibiotics sent to your chosen Alberta pharmacy. Sick note included if needed
Throat Swabs in Alberta
When the clinical picture is unclear, a throat swab confirms the diagnosis:
- Your doctor issues an Alberta Precision Laboratories (APL) requisition
- Walk in to any Alberta Precision Laboratories (APL) collection site with the requisition and your intake details
- Results typically return within 24–72 hours through Alberta Netcare
- Your doctor contacts you with the result and starts antibiotics if positive
- Rapid strep tests are not routinely used in adult primary care in Alberta — throat culture is the standard
- Children > 3 years: throat swab is appropriate when Centor criteria are met
Sick Notes and Return-to-Work / School
Your doctor can issue:
- Sick note for the duration of illness
- Return-to-work or return-to-school clearance 24 hours after starting antibiotics for confirmed or suspected strep
- School absence note for parents on behalf of a child
- Daycare clearance once fever-available and 24 hours into antibiotics
- Workplace medical certificate when employer requires one
