- Codeine-containing cough syrups — for short courses only, when warranted and no contraindications
- Inhalers for cough-variant asthma — short-acting bronchodilator (salbutamol/Ventolin) +/- inhaled cortidetailseroid
- Antibiotics ONLY when bacterial pneumonia is suspected on clinical grounds — amoxicillin, doxycycline, or azithromycin per Bugs & Drugs Alberta
- Pertussis (whooping cough) — azithromycin or clarithromycin if confirmed
- Nasal steroids and antihistamines for post-nasal drip cough
- PPI trial for cough thought to be reflux-related
- Honey and hydration for kids 1+ year (no honey under 1 year)
Red Flags — When We Refer or Send to ED
Some coughs hide pneumonia or other serious illness. Go to ED or expect an in-person referral if you have any of the following:
- Trouble breathing at rest, can't speak full sentences
- Oxygen saturation below 92% on a home pulse oximeter
- Coughing up blood (more than streaks)
- Chest pain that is sharp and worse on deep breath
- Fever > 38.5°C lasting more than 3 days
- Rust-coloured or thick purulent sputum with shaking chills
- Cough plus night sweats and unintentional weight loss (rule out TB or malignancy)
- Infant under 3 months with cough — direct ED
- Wheezing that doesn't respond to a rescue inhaler
How the Virtual Visit Works
Three steps, usually finished within the same morning or afternoon.
- Step 1 — Book a slot and complete a short intake covering duration, fever, sputum colour, breathing, smoking status, contacts with TB or pertussis, and any pulse oximeter readings if available
- Step 2 — Meet your licensed doctor by secure video. They listen to your description, may ask you to cough on camera or use a stethoscope app, and screen for pneumonia and asthma
